Are the green stones real emeralds?

No. They are green cubic zirconia chosen for the emerald green color. Emerald in the option label describes the shade, not the material.

What changes between the 3mm and 4mm option?

Only the cluster head: it measures 3x3mm or 4x4mm. The larger head is easier to read from a distance, the smaller one keeps a very fine look, and both use the same 20ga post thickness.

How should these studs be cared for?

These are pierced studs for an existing healed piercing. Take them off before bathing, swimming and applying perfume, wipe them with a soft dry cloth, and keep them away from cleaning chemicals and abrasive polishing.
